I'd just remove it permanantly tbh. its a good method of removing old medications from the water, as well as nasty stuff like ammonia. however, it has a maximum capacity, and this is usually reached within a few weeks, then it just leaches everything back into the tank in one go, thus causing more problems.
imagine it like a sponge, absorbing all the bad stuff. but then, after a few weeks, it wrings itself out, releases all the stuff its absorbed, and becomes useless.
It was used a lot in the 70s, where a lot of legends and myths were spread about it by aquatic retailers to sell more of it. sadly, even though science has proved its long-term inefectivness and dangers, some of these legends about it being amazing still find their way into advice given by shops.
just remove it forever, and replace it with ceramic "noodles", available from all good aquatic retailers.
