From the pictures you have posted this isn't typical for sarcoptic mange. Usually these dogs will be itchy mostly on the ear margins and behind the elbows and hocks, although it still is possible.
This sounds like a tough case. Usually any skin problem can be treated with antibiotics, steroids and a good flea/tick medicine. You may need to have your vet do a biopsy of the area. This will be a little pricey (in my office it's around $300 Canadian) but it is definitely the best thing to do when a dog is not responding to the normal treatments. I really hope all is well!
