It would sound plausible if all her kittens were there...
But she has been out and about with Smurfette, so why would the others be locked in, and not Smurfette. If she is able to follow mummy around, so should the others.
And she knew where her kittens were, so she would have gone through a lot of trouble getting back in, and people would have noticed her moping around. She would probably have tried to attract people's attention to get back in.
She is a barn cat used to people, not some feral cat who will not show herself to humans at all....

To me, it sounds more like she abandoned the litter, and took Smurfette with her.

Given that Lily managed to nurse these kittens fo 4 weeks at least then I think the chances of any congenital problems are very unlikely. Usually they reject those kittens very soon after birth. Sounds to me therefore the root problem lies more with the mother ( she may have an infection for instance ) than the kittens though obviously they have suffered as a result. Alternatively they may developed an infection etc in the intervening weeks.

Until you can get them to a vet, I would also check the kittens for fleas and remove them promptly with a flea comb if you notice any. Very tiny kittens can suffer from flea anaemia making them very weak indeed.

Another thing - Are you absolutely sure these kittens belong to Lily given the circumstances?

You have a point there, they might not be Lily's kittens at all, given her lack of interest in them and her devotion to Smurfette....

Do you have any other cats around, maybe ferals, who may have used the barn to give birth? A true feral would probably not come back, once the litter had been disturbed by humans.......

They don't always reject kittens straight away, though..

I remember a litter of my friend's cat, with 2 healthy kittens, another one with a bowel problem and one with down syndrome.
They were probably the result of inbreeding, she had rescued the cat from a farm when she was already pregnant, and there were quite a lot of cats there, none of them neutered, and all related in some way.

All kittens were nursed, initially, and the one with the bowel malformation was rejected by the time they started taking solid food and he couldn't digest it. The down kitten could not keep up in a lot of ways, his behaviour and movement were rather awkward, and mother lost interest in him at 5 weeks or so...
