I still cannot figure this out.

Okay, it looks to me as though this attaches to the top of your fence and does it turn when your cat tries to climb over it

If anyone has this in their garden how effective is it please? Nothing like a personal recommendation

I think I would need about 6 of them to keep my cats from getting out Don't think I would trust them plus what if my cat tried to get over and fell off

I prefer the angled fencing at the top of the fence.. Although some cats can get over that too!
