Neglected Cockatiel - How to help?

Hi there, my boyfriend has quite an old cockatiel, I think he's about 12 or 13 now. He used to be really well cared for and happy. But in more recent years his gran won't let him out the cage, he's stuck in a small cage that is only JUST big enough to stretch his wings. Sometimes I notice him puffed up on his perch, never for very long though, and I haven't seen him on the bottom of his cage. He did have a bit of crust around his nostrils but I got him out and used a moist cotton bud to get rid of it, and it hasn't come back. I also got her to change his seed mix to something with less sunflower seeds in it as the previous one had loads. I tried him with some veg but he doesn't know it's for eating :S He's only ever had seed apart from maybe some doritos when they just got him all those years ago. His vent is clean and he preens a lot, but his feathers don't look in great condition. I bought a wee spray bottle for him.
Every time I go over there he screeches and paces accross his fave perch and it's obvious he wants out.
I made a fuss a while ago about letting him out because he honestly probably hasn't been out his cage in months now. They let him out for a while but then stopped again. After me having to grab him (to clean his nostrils) he wouldn't let me anywhere near him for ages. He's only just let me pet his head again the other day.
I really worry about him though, he must be so frustrated. His cage is right at the window with no shade from bright sunshine. And right next to the radiator, I get boiling in that house so I do wonder about the birdie!

We're supposed to be moving in together at some point once he has enough money coming in, I just wish he'd hurry up so I can take the bird, and get him a bigger cage and actually let him out.
If anybody knows any specialist avian vets in Glasgow that would be great, and roughly how much it costs for a check up?
Any advice you can give me about any urgent things that need dealing with?
I've never owned a bird before so not sure what's urgent and what's just not great out of this?
