Given the history, i would recommend spaying her, and yes it can be done at the c-section. If she's having so many problems, and a phantom (they run in lines so she would maybe pass this to her puppies) its not worth putting her through it.

And judging by the conditions under which she was mated id say an urgent trip to the vets, you can take things like this lightly. If she was aggressive she obviously wasnt ready for covering
