This is what happens when you punish puppies and dogs they do it and hide it.
Go right back to basics with him and treat him as a new puppy,
You'll need lots of patience,a puppy doesn't have any control over there bladder until they are around 6 months old,so it's wrong to punish them for your mistakes,your not picking up on the signs.
Take him out every hour,after sleep,playtimes,mealtimes and stay out there with him till he performs,praise and reward when he goes.
Maybe link a word such as Be a "clean" boy whilst he is performing,so he assoicates the two together
When he Toilets Indoors,simply pick him up put him outside,don't say anything to him,Ignore what she has done & clean the area with Biological washing powder to get rid of the smell.
You will get there it just takes time & patience.
My Tyler was a peice of cake to toilet train and was clean both day and night by the time he was 5/6 months old.We also used a crate at night and still do, I believe this helped in toilet training at night.We had the crate upstairs during the night in our room,he would wake during the early hours so I would get up and take him out,extending the time by 1/2 hour each night,settle him back down,it worked for us.
